What is infertility?
Infertility is when a couple cannot get pregnant (conceive) despite having regular unprotected intercourse for at least 1 year.
Around 1 in 7 couples may have difficulty conceiving. About 80% of couples will conceive naturally within a year if they have regular unprotected sex (every 2 or 3 days).
How Does the Doctor Determine my reason for subfertility?
To determine the reason for subfertility, your doctor will ask about your medical history, medications, menstrual history, and sexual history, etc, as a primary step followed
by clinical examination.
Evaluation of both partners, male and female, simultaneously is important.
Later on, some blood tests, an ultrasound examination, and semen analysis were done depending on the condition.
What are the Common reasons for infertility?
Problems could be because of male or female or both partner-
Male factor-
Semen production abnormalities or outflow tract obstruction for semen.
Medical Disease- Diabetes, hypertension, chronic renal disease, medications etc.
Trauma, surgery, chemotherapy, radiotherapy, smoking, alcohol
Hormonal imbalance- thyroid, obesity, hyper- prolactinemia, hypothalamic and pituitary disorders
Infections- mumps, Tuberculosis, sexually transmitted infections (STD’s) etc.
Genetic disorders
Female factor-
Ovulatory dysfunction- Hyperprolactinemia, thyroid problems, obesity, Polycystic ovary disease (PCOD), etc.
Tubal factor- blocked or damaged tubes because of infection, surgery, or endometriosis.
Uterine factor- fibroids, polyps, adenomyosis, or intrauterine adhesions, Congenital urogenital development defects
Genetic disorders
Advanced age, poor ovarian reserve, or premature ovarian insufficiency (POI), surgery, smoking, etc.
Unexplained- When no obvious cause is identified.
What are the treatment options available for infertility?
Counselling and awareness about the fertility window-
Sometimes, only making a couple aware fertility window (5 days before ovulation till the day of ovulation) helps them in achieving conception.
Ovulation induction (OI)- Use of fertility drugs to achieve egg formation.
Intra-uterine insemination (IUI)- Insemination of a prepared semen sample inside the uterus (womb) near the ovulation (egg rupture).
In-vitro fertilisation (IVF)/ Intra cytoplasmic sperm Injection ICSI- After stimulation of ovaries with medicine, mature eggs are aspirated and inseminated/injected with sperm to create an embryo inside the laboratory, which are transferred back to the uterus of cryopreserved for future use.
Third party reproduction- oocyte or sperm donor and

Fertility-enhancing surgeries- Done through an endoscopy approach, eg, laproscopy/hysteroscopy/both, like polypectomy, myomectomy, septal resection, metroplasty, adhesiolysis, cystectomy, etc.
Is IVF Painful procedure?
No, IVF is not a painful procedure. It involves the administration of injections, which are mostly subcutaneous, like insulin injection, and transvaginal ultrasound monitoring of egg formation. Then eggs are aspirated under ultrasound guidance with a very fine needle through transvaginal route and its a day careprocedure (same day within 4 hours, allowed to go home).
Does complete bed rest require after IVF?
No, it’s a myth that complete bed rest improves IVF results. In fact, keeping yourself active and performing light activity helps in reducing anxiety & maintaining good circulation.
What is the success rate in IVF?
Success rate of IVF depends on many factors-
Maternal age is the most important factor; increasing age has a negative effect on IVF success.
Quality of Egg, Sperm, and Embryo.
Previous Pregnancy/abortion history.
Male factor
Medical condition
Previous uterine surgery, genital infections, etc.
Lifestyle factors
